Fever and Fatigue

Description: 

This "Fever and Fatigue" virtual patient allows you to explore a history of present illness, gather pertinent physical signs and work with a virtual Electronic Medical Record (EMR) as your order and review investigations. You will adjust your sense of possible etiologies as you move through the case, with opportunity to compare your perceptions to those of other students, residents and clinicians.

This Virtual Patient Case was submitted as part of the CHEC-CESC Virtual Patient Challenge 2012 as part of the AFMC-Infoway Physician in Training e-Health Curriculum and e-Learning initiative.

The focus of the challenge is on virtual patients that incorporate the use of electronic health records (EHRs) and other aspects of e-health to improve patient care. John's Back Again

Description: 

 

Low back pain is one of the most common presentations in the Family Practice. In many patients, the low back pain is self-limiting. If there are no red flags, the patient can initially be treated with simple pain killers. However, of the patients seeking medical attention, many are unnecessarily being sent for further investigations, which contributes to high health care costs.
This case "John's back again" was developed for students to learn tips on diagnosing and managing low back pain, including exploring red flags symptoms. In addition, this case will give them an idea on how quickly health care costs can rise by ordering unnecessary tests. Furthermore, it shows them how Electronic Medical Records (EMRs) and network systems operate and how they can be helpful and time-saving in daily practice.
For the authors, this case gives them an opportunity to see how feedback and instructions influence the student's choices in physical exam and management plan during John's second visit to their office.
Students can get direct feedback during the case and get an overall summary on their performance at the end of the case.

Bob's Blues

Description: 

The Case presented features a patient with depression resultant from a combination of social, genetic, and addiction issues. You are his family physician evaluating him for these issues. You begin by reviewing your EMR and the provincial EMR. Continuing, you investigate his complaints including social issues such a caregiver burnout. You are then further lead to make a diagnosis and treat his underlying addiction issues. Outcomes are based upon your ability to relate to the patient, offer a solution to his caregiver burnout, choose proper medications and appropriately address his alcoholism.

Bleeding Barbara

Description: 

Abnormal Uterine Bleeding is a common patient concern seen in primary care. Bleeding Barbara’s virtual case will help clinical clerks and residents to recognize AUB, learn the etiology of AUB in premenopausal women, define various patterns of vaginal bleeding, learn tips on diagnosing and investigating and managing AUB. This case also demonstrates how the use of information systems such as electronic medical records (EMRs) and personal health records will influence how we practice.

Unforeseen Circumstances

Description: 

After completing this case, the fourth-year medical student is expected to demonstrate the following CanMEDS competencies:

  1. Perform a complete and appropriate assessment of a patient with diabetes mellitus type 2 (DM2), with emphasis on general aspects of screening, diagnosis, initial management using metformin, and screening for complications. (medical expert)
  2. Safely and effectively communicate medical information to patients with the appropriate use of e-health resources. (communicator, manager, professional)
  3. Collaborate effectively with health professionals in an interprofessional diabetes team care setting. (collaborator)
  4. Retrieve evidence on diabetes including the latest Canadian guidelines and evidence-based physical examination techniques. (scholar)
  5. Appropriately utilize an electronic medical record (EMR) to review laboratory results and team-based progress notes. (manager)

An Abdominal Brief

Description: 

At the end of this case, the student should:

  • Know all the components of a complete history and how to elicit them.
  • Be comprehensively knowledgeable about biliary colic, acute cholecystitis, acute cholangitis, and acute pancreatitis;
    • Risk factors and pathophysiology
    • Signs and symptoms
    • Laboratory and imaging studies
    • Management and treatment plans
  • Know how to maximize effective communication and teamwork to enhance patient care; e.g. the use of consults, communication tools such as SBAR and SIGNOUT?, and patient-centered inter-professional pre-operative assessments.
  • Be comfortable with electronic health records, while being aware of patient safety, privacy and confidentiality issues.

A Fatigued Figure

Description: 

By the end of the virtual patient simulation, the learner should be able to:

  • Identify the pertinent questions, physical examinations, and investigations needed to diagnose a patient who presents with fatigue.
  • Demonstrate the skills needed to manage medical conditions such as anemia, menorrhagia and neoplasm.
  • Demonstrate the incorporation of electronic medical records (EMR) in patient care.
  • Demonstrate effective management skills through the judicious and cost-effective use of tests and resources.
  • Demonstrate an understanding of inter- and intra-office collaboration in patient care between professionals including, but not limited to, nurses, family physicians, specialists, nutritionists, and therapists.

Poppy Gone: Prenatal Care

Description: 

This case is part of a series being generated for the CFPC SharcFM series.

This particular case deals only with the very early issues in a pregnancy. Follow along with Poppy's pregnancy as she goes through a series of prenatal adventures. While each case will be standalone, there is also benefit to exploring the case series longitudinally to see how her pregnancy unfolds. 

Learning Objectives:

-- Explore some common issues in early prenatal care
-- Develop awareness of costs of common investigations
-- Demonstrate critical thinking around common screening measures
-- Develop awareness of how use of an EMR can improve practice

Target learner groups: primarily late stage medical students, Clerks and early Residents. Clinical faculty may want to use the issues raised in this case to explore some other teaching issues, such as around ethics.
